Buckeye Community School, through it's board of education, management company, staff, and partnerships with key stakeholders including, but not limited to parents, business advisory councils, family and civic engagement teams, school-parent or school-business partnerships and other schools, social services and organizations that impact education programs for students, is working to ensure that all students, family members, and friends feel safe and supported during their time at BCS. Household Information Survey
Buckeye Community School participates in the Community Eligibility Provision (CEP) under the National School Lunch Program (NSLP). Under this option, all children in the school receive a breakfast/lunch at no charge. However, to determine eligibility for various additional state and federal program benefits that BCS may qualify for, please complete, sign and submit this form. This survey needs to be submitted each school year.
Emergency Medical Form
The Emergency Medical Form is required to be updated every school year.
